In order for your child to take part in our activities, Titan Sports Academy need to collect their basic details, including contact information and relevant health information.  We will only use this information to help us provide the activity, look after your child and contact you if necessary about your child.

If your child has a medical condition, you should seek medical advice before allowing them to participate in our activities.  Whilst all our Coaches have liability insurance and will take every precaution to ensure that accidents do not happen, they cannot necessarily be held responsible for any loss, damage or injury suffered.

By agreeing to these terms and conditions, you are giving permission for the organisers to sign on your behalf for any emergency medical treatment that the named person may require on the understanding that every effort shall be made to contact you first.

Our booking form includes an optional Photo Consent check box.  We would appreciate parents' consent allowing us to use photos or videos taken during the activity to promote the programme via our website, social media or leaflets.

We will never share your information with third parties unless it a matter relating to the safe guarding of a child, young person or vulnerable adult.
